Adoptive Mother - Question *SPOILERS*

I don't understand the adoptive mother saying something about getting 50 dollars U.S. per month for a baby. Whose baby was that? The adoptive mothers? She looked too old to have children. Or was that a flashback to when the protagonist was born? And if so, can someone explain further what that scene meant, coming at the very end as it did?
