I'm assuming it was syphilis or a similar STD. This was '48 and people were still dying from that then, most famously Capone a few short years earlier. It's visible so he mentioned how he couldn't undress in front of Kitty his wife even though she wanted to have kids. Also the petrie dish Nicholson looked at had tiny tadpole like creatures swimming in it.
