Rusty said something along the lines of reaching out to her with murderous rage(?) Did that mean he wanted to kill her, or did kill her?

nothing ambigous here.he said "i reached for carolyn and set off that insane mix of rage and lunacy that led one human being being to kill another." meaning,by having the affair with carolyn,he prompted his wife to kill her.

Joshua is absolutely correct. It was not at all ambiguous.
