Need help figuring out what a (possibly) similar movie was.

Sometime in the 90's, I saw a movie on HBO and I've been trying off and on over the years to figure out what it was.

I can't remember much about the plot, but it seems like it had a society of freaks living underground, similar to Nightbreed. I have only one clear memory of a scene from it. In this particular scene, the main character is being escorted through some sort of settlement inside of caves, and there were lots of various types of freaks/demons/creatures in it... and at one point they come upon this enormous, naked fat woman. This fat woman then lifts up her giant breasts and there are these agonized faces growing out of the underside of them, like trapped souls or maybe she absorbed them into her body or something.

I've seen gifs of this pop up in random places on the internet from time to time, yet I've still never managed to figure out what the *beep* that movie was. For all I know, it could have been some weird cut of Nightbreed that I hadn't seen before or since.
