How did he die/get infected? He kind of looked like a bald Wilford Brimley.

Everyone who dies comes back in Romero's movies, not just those bitten. Thousands of people die of natural causes every day. Rege could be a heart attack.

I wonder why we never saw the cousin who blew his head off come back and Judy and Tom (even though they got burned). I know they could have and we just didn't get to see them.

You answered your own question about the cousin - "blew his own head off". Remember you put down a zombie by destroying the brain. In this case the brain was already destroyed.

As for Tom and Judy - Both may have had their brains fried in the explosion and thus why they couldn't reanimate.
