Well, if she would have said she either 1: Got a divorce and was signing her married name, or 2: She just got married and was signing her married name instead of her maiden name. She could have used any reason, but it is weird they never questioned it.

Seems the film makers should have said SOMETHING instead of just glossing over it.

It's supposed to be a comedy moment. Saying "I signed the wrong name" is pretty much the WORST thing you could say when trying to get 4m Dollars out of a bank account that isn't yours! The point of that line isn't how the people at the bank react to it, it's Sam's exasperated reaction.

And, Patrick Swayze nailed that exasperated expression -- and again when Oda Mae said that she had a transfusion to make!

I always thought they assumed that she meant that she had changed her name and accidentally reverted to her old one, or as others have said she had a married name.

Whatever the case, it was funny.
