confused about one thin

why did bill pullman start going crazy after he did the operation on halsey? bill pullman must have never even done that operation at all right? and he must have been psychotic the whole time and simply imagined doing to the operation on halsey?

also, just want to add that this movie is awesome and highly underrated. and pretty damn scary at times, like when halsey is in the closet with his eyes missing.
