I recently bought the Israeli release of the Yossi & Jagger DVD and was pleased to find that After is one of the extras on the disc. The Israeli Yossi & Jagger disc does have English subtitles. I bought it here:

It's a Region 2 disc (PAL)
2.0 Stereo
4:3 Full Screen Format
Language: Hebrew (with English subtitles)

The site is reliable, I've also used it to get the Eytan Fox film "Walking On Water"

It's also availabe as one of the four films on Boy Life 5. The other three movies are unremarkable. So, if it's included with Yossi and Jaggar I would buy that over Boys Life 5.
