Did she die at the end?
At the end it showed Carol collapsing and the movie ended. Do you think she died?

No, Carol did not die. She was pretty banged up so I've always thought she just collapsed out of pure exhaustion after what she had just been through. I probably would've fell to the ground too. LOL
If you have the DVD there is an alternate ending after the church, presumably, some time later, with Carol and Todd playing the the park showing they both recovered and was able to get on with their lives. I remember this scene airing on network TV but it wasn't in the original VHS release, but it's in the special features portion of the DVD along with some other deleted scenes.
I remember seeing that alternate ending on TV as well. I love the Special Edition dvd because it has that alternate ending and other alternate/deleted scenes.
