A couple of questions about the movie (spoiler)

1] In the scene when they are sitting in the club waiting to bust the drug dealers, Kikuchi asks Azuma why his sister was in a hospital, he answers that she has problems with her head and Kikuchi says something like "oh, not again" and they laugh. Anyone can explain what does that mean? Maybe I'm leaving out some words which make all the difference but I didn't understand the meaning in the first place so I cannot remember it correctly.

2] Was Azuma's sister already an addict who just proposed for sex in exchange for dope, or did the pushers kidnap her and start to drug her massively in order to have her under control and rape her comfortably?

I just watched this film.

1) In that scene, when Kikuchi asks what was wrong with Azuma's sister, he replies "her head" which Kikuchi thought was a joke about his sister being stupid (I think). I'm not sure if Azuma really meant that to be a joke or if she really had some sort of head injury...

2) She seemed pretty normal before being kidnapped and raped so I think she got addicted after being injected the *beep* by the rapists.

I took that the problem of the sister are psychological (she seems a little crazy) and that's why he said "her head" but I found strange that Kikuchi laughed at his face about that, seeing how japanese seem to have very good manners
