I remember watching this on vhs many years ago and remembering it to be a bit sick, but I saw it recently and there was nothing to it. I must be getting old.

Horror reflects the standards for what we consider violent. After all the beheadings, massacres, rapes etc that we see on television from real life and fiction this would drop in effectiveness. Even Saw is dropping in standards.
