Confused about ending (Spoilers)

At the ending, when Sarah and her boyfriend are in the car, and Sarah is impaled with the hand, does that mean they're still in hell?

Not hell but this. Mary Lou supposedly did some kind of magic making Alex & Sarah go back in time in the 1950s. Mary Lou kills Sarah. I don't know if Alex was a ghost or not cuz when he was asking help from the people in the diner, they didn't see him. Thus, he was now stuck in Mary Lou's time forever

I thought they were still in Hell. Alex was begging people for help and they didn't hear him so I assumed they were just the spirits of the dead.

I read it as an "ironic hell/purgatory" that Mary Lou had control over since it resembled the 50's setting she lived in.
