You don' just choke to death in your sleep, I assume. So even if she fell asleep at the table with food in her mouth, and it slid down her throat & got stuck...she'd wake instantly and flail around.
Why did no one see her do this??

She actually did wake and flail around, it transitioned to the real world and showed her choking and everyone just looking at her (though I'm not sure on what since you don't tend to eat while sleeping). Usually they try and transition what happened in the dream realm to the real world (like making it look like Shelia had an asthma attack in part 4). But I'm not sure what they would have said Greta choked on while she was asleep at the table.
