Was there another horror film that had a similar cover, except that the characters looked more like the painting and just had evil expressions on their faces? I thought I saw this before, but I could merely be thinking of this film.

The VHS and DVD have different covers. I'm not sure which one you're looking for.
The original poster/VHS cover look more like the American Gothic painting. The newer DVD cover, for whatever reason, was slightly changed.

I thought the same thing too. But it could've been a book cover or even just some art someone did to make the painting really scary.
