Yo no comprendo

I didn’t understand this film. I watched with Danish subtitles and some of the words are unknown to me. Why did he self harm? And cut her eye out? Did she write the novel? Why did he smother her and then became
transvestite. Like a Norman Bates but it’s girlfriend and not mother.
