Intro question

Does anyone know how many versions of the intro are there? I know that there's the original intro that was used during the first season, as well as the 2nd intro that came after they re-modeled the playhouse in the second season of the show, and a 3rd intro that was used from seasons 3-5, but are there any others? Also, are all versions of the intro on the DVDs?
