This Scene Stinks

Why was Holmes dressed up in that clownish get up in his last scene in the movie? Does Conan Doyle ever specify that the soft cap that SH wore was actually a deerstalker cap?

No. The Stalker hat is purely an in invention of the artist Sidney Paget who illustrated 'The Strand' stories.

The last shot of Holmes is to show what he would eventually look like when he was older and the Holmes we knew, with his pipe, coat, and hat.

Oh, PLEASE!!

That's one of my favorite scenes (I recall bursting into laughter the first time I saw it, it was so wonderfully playful and creative)...

And all of the elements relate to events earlier in the film: the cloak was Rathe's; the cap belonged to Waxflatter, his mentor, and the pipe was purchased by Watson when they were looking for info regarding the blowpipe.
