You mean in the hospital? That's Malfeitor (although how she sees him without having any prior knowledge of him is a good question - maybe he was so powerful at that point he could actually appear visually as a ghost).
At the end she probably dresses like a guy because Malfeitor is/was a guy and he wanted her to look the part.
