Just because that's the ONLY scene for Celie doesn't mean it was the only time for SHUG. Do you see how shy and childlike Celie was with the kissing? Shug was a pro at it so it couldn't be her first time at a rodeo. Steven Spielberg says his only regret is that he didn't explore the lesbianism more in the film.

It was assumed that they were referring to an STD.
But I think it was just a rumor spread around by angry/jealous folk in the film that Shug had an STD, as she is seen having sex with Albert, Celie, and her husband with none of them catching anything.
