ID episode w/ sex-changing antagonist?

Forgive me, for it has been almost 30 years. Used to watch this since our family had HBO even before we had basic cable.

One episode I vaguely remember had this guy running for his life down the alley. Slowly he starts turning into a woman.

Anyone have a brief summary on this episode? That's the only scene I remember. And I remember the one with the drug-using rockstar who sucks in 2 nosy reporters, and the bad plastic surgeon one.
