In the Behind the Scenes feature on the Kill Bill 1 dvd, Tarentino states that he took the character from a series starring Sonny Chiba. I think it was something like kage no gundan, or something. Also, Hattori Hanzo was a real person.

Quentin Tarantino had Sonny Chiba play the character Hattori Hanzô which he also played in a television series called Hattori Hanzô: Kage no Gundan.
It just happened that there's also a character in Shogun's Ninja called Hanzô Hattori. share
Hattori Hanzo was also the name of a 16th century ninja master, but he would have been about 12 years old at the time of this film's setting.
