How does that German couple at the beginning add up to whole? Were they killed on the same island like the rest or what?

Yes they were. At the beginnig of the movie during the credit sequences you see them walking through the later abandoned Village on the island (shot in Sperlonga, Lazio, Italy)

So, D'Amato over Deodato, at least D'Amato doesn't have the *beep* dog killed on camera and eaten or something.
