Why is Ian Hendry's character not credited

Does anyone know why Ian Hendry's character (Hitchens) is not credited? He had a bigger part than some of the other characters like Jamie Foreman (Driver). Was it an oversight when the credits were made or did Ian Hendry ask to be removed from the list?

Often wondered about this myself, the "trivia" only mentions that this was his last film - perhaps his health was failing? Just a guess

He looks unrecognised as Ian hendry looks in very bad health too his performance is rough
