How did City Of The Walking Dead get just an R rating?

I've seen City Of The Walking Dead on 3 different home video releases: Continental Video's VHS, New Star Entertainment's VHS, and the DVD double feature with Creepers. All 3 prints were the unedited Nightmare City print with the City Of The Walking Dead title card. On these title cards, there is an R rating symbol printed on them.

How in the world did City Of The Walking Dead pass with just an R rating? How could it possibly pass without any edits?
