Because he refused to sodomize Patroclus, hence why the scene was changed to fisting.

The issue was the law. McDowell initially asked Vidal to rewrite that scene, and to imply the rape rather than to show it. Vidal wouldn’t budge. McDowell did everything in his power to avoid performing that scene. Because he was homophobic? Hardly! In 1976 any actor who were to perform such a scene, even simulated, put himself at risk of arrest for indecency. It was producer Franco Rossellini, an avid participant in Manhattan’s nightclub scene, who brainstormed a replacement scene, based on something he had just witnessed at an underground club called ‘The Toilet’: fisting. It was a new concept to him and to McDowell. It was a compromise that McDowell could live with.

The similar risqué scenes that Tinto Brass shot (the beauty cream and the two guards behind the moon mask) included McDowell, but not as a participant, and so he was okay with those.

P.S. I should add that once the laws were relaxed, McDowell felt free to perform such scenes, as we can see from I’ll Sleep When I’m Dead (2003, http://www.imdb.com/title/tt0319531/?ref_=fn_tt_tt_2).
