The instrument the kid is playing is called "Češalj" which means comb in Serbian (yes, a simple comb for combing the hair). It is mostly a supporting instrument, which can be played single but usually is being played as accompanied with the accordion. It is popular in Serbian and Balkan music, but I've seen some southern Italian kids playing, too.
shareEhem... the instrument in the movie is actually "drombulje" - and the English name is "Jew's harp" (I was not aware of the latter until I've looked it up just now).
