Hobbs in the film is based on two characters from the book, Hobbs and Ellis. One of the themes in the book is that daily contact with mental patients can bring out your own mental illnesses, which everyone has, but some people can't handle them. Hobbs was recognized by the patients as being as mentally sick or sicker than they were and it terrified him. That's why he killed himself. Ellis was his replacement and also afraid of the patients and himself. Deborah witnessed Ellis beat Helene while she was tied up in a cold pack. She reported it, but Dr. Fried warned her the administrative staff might not do anything about it, which is why it's called "I never promised you a rose garden", i.e. reality isn't always perfect or fair.

In the film it's all simplified. Deborah witnesses Hobbs beating Miss Coral. She runs for McPherson, a good attendant who is also from the book (and beautifully played by Norman Alden, who's obviously read the book). She speaks to him urgently in Yri and takes him to where Hobbs is. Hobbs kills himself after being dismissed for this incident.
