Okay, the dwarf in this film is the only one I've ever seen naked... but is it normal for the base of the spine to be so significantly curved as his was? It looked terribly painful to me. Is that a common occurrence for dwarves and midgets to have horribly disfigured backbones?

There are many different forms of dwarfism. The most common form is called achondroplasia, which often involves some degree of spinal curvature, either convex (kyphosis) or concave (lordosis). The man in that scene appeared to have extreme kyphosis; far more than in most people with dwarfism.
