significance of the monogramed gold lighter?

Stefano seemed to think Francesca's gold lighter was important. It had the painter's initials on it. Coincidence (red herring) or was Francesca somehow involved with the painter and his sisters? (a daughter, perhaps). Seems like the film failed to follow it up so either it was a red herring or I missed something.

Nope, they just let it turn into a dead end. Francesca says "it was a gift", then "it belongs to a friend", but since she arrived on the scene same time as Stefano -- was on the same ferry boat -- she could not have had any prior exposure to the characters in the town. It's possible that the initials on the lighter served as an intentional red herring to suggest more plot intrigue than was actually written....

And why was Francessca wearing Granny-Panties?

The lighter had my initials. So, yeah I was kind of wanting to see how it connected too. Oh well. Great movie all around.
