Question about Movie

Years back I saw a movie about a couple of people ship wrecked on an island. Of course, they at first do not like each other but learn to live with each other based on their circumstances and somewhat fall in love. I want to say the woman is some sort of princess or something or part of a royal family. And the actress playing her is someone that is somewhat famous. In any event, the things that stick out to me the most is how the two view each other over time as being healthy, beautiful, etc. but the camera also flashes to their bodies and how they really look (emancipated, starved, etc.). This probably does not make much sense, but there is very little I can remember about it. I have seen Swept Away some time ago, but I don't remember it striking me as the movie I am talking about. Does anyone have any idea what I may be talking about? I seem to remember a video cover of the woman standing on the beach, hands over head, with some sort of scarf blowing in the wind.
