What was the whole final scene all about? How did white afro guy end up in the morgue and why did Cippriani's wife get all freaked out when he denied knowing who the guy was?

Yeah, I had no idea what that final scene was about either. I don't remember hearing Sergio Sollima say anything about it either.

Here is my take on the ending (watched it last night for the second time and this is what I came up with):

She is freaked out because she can't believe that Vito isn't putting the blame on the guy who kidnapped her and put her through hell (made her take drugs, etc). I think that the cops actually killed the white afro guy and wanted Vito to back up the story that white afro guy was responsible, which he was. Vito refused to go along with them because it was their fault (by not supporting his confessions) that he had to kill Milo.

Having just watched this again, I have a different take on the ending.

The powers-that-be want the whole political assassination to be buried. At the end, everyone who knows about it is dead except for Oliver Reed. By denying knowing the cadaver, he is showing to those powers that the secret is indeed safe. His interviews at the end with the various officials tell him they know what is going on and that they want the issue resolved. They will leave him and his wife alone if he remains quiet. His wife is shocked because she knows the body is that of her kidnapper, and, whether or not she knows about the assassination, she is surprised at the compromising of her husband's principles.

Italian movies of this period often had political overtones. Fabio Testi dies right when he becomes socially aware and wants to expose the plot to the papers, and Oliver Reed lives but becomes part of the corrupt establishment.
