I might have seen this movie on tv when I was five if he did.

I have this on DVD, I didn't see him drive an ambulance. After Robert Duvall's character shoots James Caan during the first part of the movie, an ambulance takes Caan to the hospital.

You're not thinking of Bobby Duvall in The Outfit (1973), a similar but superior film to the Peckinpah?

James Caan didn't drive an ambulance in "Freebie and the Bean" though. But he was a passenger.
