Tony the pimp was providing Madelaine with ultra pure heroin. Heroin in it's pure form does not need to be "cooked up", or heated over a flame. The way "one Eye" mixes the dope in a glass is a bit peculiar, however that method would work fine. Great film though, huh?

Another thing I noticed whas the absence of the classical rope/rubber-tube around the arm prior taking the "shot"...

I also suspect that the effects of trying to get off heroin where slightly exagirated for dramatic purposes.

but make no mistake... heroin is bad in every form, ok? ;)

The heroin use in the film is extremely unrealistic, as is most of the film.

As for the getting off heroin part being unreal, that's one of the more realistic parts. 7 out of 10 people who try heroin are addicted for life. Most addictive drug around. The effects of the drug were not shown well at all in the film however. Try Trainspotting or Sid & Nancy if you want to see realistic heroin use.

Rope/rubber tube not needed unless your veins are hard to get at. It's harder to find good spots to inject the more you use, so people who have been junkies for a while are more likely to need a tie down to perk their veins.
