I heard it was Robert De Niro, but there was some sort of dispute with the studio, so all the shots of his face were cut before the movie's release.

Alfredo was played by Nico De Silva, the face in the locket.

What's weird is that the head in the locket, when it's first shown to Oates, looks like Warren Oates.

He would have been 8 years old then, but he is a good actor, so maybe....... :-)
