Why did *SPOILER* actually die?

Who killed John in the end?
Was it really a midget? If it was, why did she wear red, like their daughter?
Was it death personified?
The only logical explanation I can come up with is the midget is the personification of guilt and grief, that eventually kills him - but he doesn't really die from a stab; we're just not told how he actually dies.
Oooor... it's the serial killer, which we don't really see, but John sees him as his daughter, because grief has taken over his mind.
Or?...

MAJOR SPOILERS OF COURSE

It was really a midget who killed him - she was the serial killer.
She was dressed in red, yes, but he thought, or hoped he'd see his daughter.
Or at least to discover some truth. Dunno for sure. He sure hoped something
along that lines, even though he refused to admit that, but didn't hope for
what he got.
When someone cuts your throat like that, and you locked yourself out
of help I'm pretty sure you'll die by the time you reach to hospital.
So that WAS the reason of his death. Don't you think that every killer is in a way death personified.

What is kinda funny is that his gift was his demise.
That alone is unique idea that one can appreciate.

The film suggests that by repressing his psychic abilities he fails
to save his child in time, and eventually gets himself killed.

Someone said - In HINDSIGHT this movie is brilliant.
Well, maybe not brilliant, but really really well done.

Oh, the humanity...

Yes, something like that.
Still, I would be bothered by the idea that the killer wore red, just like his duaghter... It would seem a convenient coincidence.

Nice one the "repressing his psychic abilities he fails
to save his child" piece ;)

I'd say it was pretty inconvenient for him. It's probably inconvenient for the dwarf too because for all we know she was out for a stroll in her nice pretty new coat when some crazy American guy started chasing her, before locking her in a room. For all she knew he was the serial killer terrorizing Venice and was going to rape and murder her too, so it's a good job she'd brought her meat cleaver along for protection. She's still going to need a good lawyer though after slicing up an American tourist.

Yah, which is why they chose to tell this story and not the ones were this coincidence did not happen. Red riding hood killer does not seem that far fetched, she would look like an innocent child before they knew it was the serial killer, it could of been a blue one, a yellow one, a green one, or a red one, maybe black or white. Honestly not far fetched.
