On the first time of viewing, did you guess Dr. Masters was a patient? I must confess, I didn't.

No, I was completely shocked. I thought she was a real doctor, but just a grouchy, old one.

Yes. If you paid close attention, there were enough clues throughout the movie that indicated she was a patient.

There were a lot of clues, but I didn't pick up on them until my second viewing.

This film is actually better the second time you watch it.

I don't think I got it until the second viewing, either, although I may have guessed when she made Sarge hold that patient file (hers) and she set it on fire in his hand.

I think originally she was a real doctor (before the story) that went wacko (just prior to the story) and became a patient. She seemed to doctorly not to be a trained doctor.

"Italy invented the pizza, took it to New York...but Chicago perfected it.".

She reveals the answer herself in her speech at the end of the movie. She says something like "I wanted to be a good doctor, and I could have been, except for one insignificant life...!" So she was either a med student or a doctor who made a mistake and somebody died, and she had a breakdown after her dreams of becoming a doctor were dashed.

No, I understood that she was a patient when I watched the film. What I meant was, did you guess it before it was revealed?

No, she was never a real doctor. There was a part where the chick that was trying to screw everyone said she wanted to pretend she was a nurse and the doctor let her indulge her fantasy.

And I think the real doctor eventually kinda lost his mind because in the flash back when the Popsicle dude was remembering his surgery she was there assisting Dr. Stephens.

I thought she set up the kill by telling him to kill the doctor so she could have full power over the sanatorium. But I did not guess that she was a patient, that was pretty disturbing

Yes, I guessed it about 10 min in. When she's telling the new nurse that this place isn't run like any other hospital and she and the Dr. Stephens consider everyone family...no mental health facility would ever be run with that kind of mentality. That's just insane (pun intended).

Nope, I saw it when I was 14 and didn't realize the situation until well near the end of the movie.
