When Holly tells Cato her joke about the naked man in the mental hospital... I followed the whole thing but didnt really get the punch line or why it was meant to be funny, although I suspect the joke is just not registering because I was not alive during this time period. Could someone enlighten me please?

not much of a joke but its just based on the idea that people in mental hospitals have their own form of logic. It makes sense to them.
Logic is subjective.

The punch line is that even though people rarely came over he wore the hat and gloves in case there was company.
