Get Smart had puzzle pieces preceding and following the commercial breaks in its 4th and 5th seasons.

Another show that had puzzle pieces for the open was The Most Deadly Game, it only lasted half a season and was on in the 1970 season. Starred Ralph Bellamy, George Maharis and the lovely Yvette Mimieux. I was lucky to find a dealer of rare TV shows and he had a couple of episodes that I purchased. I don't think it has been shown since it's original run. It only lasted thirteen episodes.

Yes - very clever sequences with the puzzle pieces. And the theme song is perfect - the flute or clarinet main melody always reminds me of "thinking logically trying to make all the pieces fit". Or something like that.

The opening credits used the puzzle piece transitions for the entire run, but only the first two episodes of the series used those transitions to open each scene.
