How did Maria's husband know of her infidelity?

How could Joakim tell that Maria was cheating on him, that is I am assuming that is why he stabbed himself?

Maria's husband was no doubt aware of his wife's intimacy with the doctor before he married her. When she told him that the doctor had stayed in the spare room when he was away, he knew that they had had relations that night.
