What is the deal with that strange woman at the beginning who is staring at bruce for a a long time right in his face for no apparent reason, seems to be an unexplained scene.

For all we know she's a cougar looking for some young game.
But I think its a snobbish thing. That was the point of the humour in it.
In the end we all know Bruce is superior.

I thought that it was hilarious that Lee's character, who was such a lethal hand-to-hand fighter, squirmed when stared at by a little old lady. Which maybe is the point of that scene.

The staring lady scene is there for humour only. Tang Lung is a coy young men who travelled half the world around to get to Rome, Italy - totally away from his culture, where it is most rude to stare at someone... And this lady, waiting for her husband to pick her up at the mezzanine of the airport, simply can't stop looking at this strange "Little Dragon"! The Chinese presence was already intense in some of the larger American cities, but not so in Rome or other European cities at the time. I do not think there is a racist commentary there, only of cultural clash - and bad manners - that are subsequently explored in Tang's gesture towards the child eating icecream, and his inability to deal with the waitress in the restaurant.
shareThis was obviously done in good humor. Actually the whole first 15min of the movie is Tang Lung showing his humorous side. You can take this scene a couple of ways.
From the old lady's perspective, she has never seen a Chinese man before, so she's very curious as to what he's about. Because she's obviously rich, she's most likely looking down on him and thinking he should be working a kitchen or on a plantation. She's obviously thinks he's out of place.
Tang is in a new land and doesn't understand the Italian culture. So he's trying not to offend anyone. He obviously doesn't understand why the woman is starring at him. He's a little nervious to ask why. He's relieve when the man comes and picks her up. I think that pretty much explains it.

Hey guys ive collected a few different versions of this film and the sound does vary on them some of them are english dubbed with very poor sound quality and some are undubbed with great quality and on one of them you can hear an odd sound of lee's belly grumbling, further indicated by him covering his stomach with his bag the whole time this little gnome is staring at him. I think all points here are valid i just wanted to chime in with something i noticed which had not been mentioned before. Also explains why he wanted that kids ice cream and why he devoured 5 bowls of soup.
