Almost funny...

It's almost funny how no one recognized that Blake and the doctor were one and the same. It's like nobody ever recognized that Clark Kent was Superman because of his glasses...apparently no one recognized Blake because of his yellowed teeth???

That's also a problem in the book in some ways.

In the book no one notices what is odd about him (he looks like a meaner, shorter Jekyll) or who he is (for the most part).

There is no facial description of Jekyll and Hyde anywhere in the book just that Jekyll is tall and middle aged and Hyde is shorter and younger, when Jekyll becomes Hyde he doesn't turn into something that looks neanderthal or reptilian or animal like or anything monstrous he simply becomes more degenerate, more revolting, more cruel but simply a human being.

Coming back years later...

It should be noted that his friends don't recognise him because their minds can't comprehend their good friend doing so horrible things and being so ghastly.

Lee's face is indeed different as Blake - the make-up has him with a more puffy look - his nose looks broken/misshapen (squashed) brutish - hair unkempt and bushy eyebrows and his horrid dentistry as you mention - very effective somewhere between John Barrymore's less is more and Spencer Tracy's 'nice try' look but I still care for Fredric March's simian fiend make-up (all give fine turns in a dual role nonetheless)
