Not her mother?

I missed the first half hour or so so I am not sure if it is mentioned but in hospital old woman is told by daughter that she is not really her mother and has always treated her as such. Old woman replies that she saved her. What is the back story as I thought they were biological mom and daughter?

In an earlier scene, it is revealed that the old woman adopted Maura when she was 15. I don't recall any details, other than that.
