Even though she was top-billed, it was more or less a supporting role. The reason that she was billed first was probably because she was coming off an Best Actress Oscar nomination and a Golden Globe Award for "A Patch of Blue" (1965) - and she was currently starring in the newly-released Sidney Lumet film "The Group" (1966).

Keep in mind, however, that EVERYBODY in this film except Peter Kastner has a supporting role. He's the main character, the story very firmly centers on him, and everybody else is a member of a supporting-ensemble cast that circles him. There's no real female lead; the closest you could get would probably be Harman's character (tho you could also argue Black's), or else you could say that the "female lead" is the collective "woman in Bernard's life" -- composed of the Hartman, Black, Page and Harris characters.
