The ending (SPOILER ALERT)

We see him wrap up a large knife in a towel and take it with him, knowing what he intends to do. He then attacks a woman in the street and is arrested and searched at police headquarters. The police should have found the knife at that point and taken it away before releasing him into his doctor's custody. But then he has it at the end.

Yes, I agree that it's unrealistic. But I think by this point the film has pretty much tossed realism out the window and is following something closer to dream or fantasy logic. Maybe someone who has read the novel can comment on how it's handled there.

I would agree it's a bit of a stupid error. I have no problem with the perfect mask, it's a plot device, but the cops specifically mentioning they are searching him makes it a bit glaring. Still, the masked crowd by far makes up for it. :)

The police do return something to the Doctor saying "These are personal effects" in one scene. Maybe they didn't suspect he would do anything with the knife for the Doc tells them on the phone that "He is not violent".
