I think this is another suggestion of oral sex, which is suggested with the line about Tatiana's mouth being big enough for Bond.
Was this also someone making fun of Anita Eckberg? Were there rumors about her giving oral sex?

The same production team was responsible for the movie advertised on the poster, so it was also something of an in-joke.
shareIn the book it was Marilyn Monroe in the the poster of her movie Niagara (1953). I assume that since she had tragically died before that, they used another actress in place. And Eckberg was very popular in Europe back then.
Also, in the book Klebb is neither heterosexual or homosexual. I think he called her a sexual neuter (I haven't read it in years), but she was someone who didn't want sex with anybody. They certainly picked the right actress to play her part.
