Yes, it was obvious. In rural areas in Spain you can still see women with this sort of moustache, so I assume that decades ago it was rather common also in south Italy (very much influenced *occupied* by Spain for centuries). As a south european that has lived for some time in the US, I could image how weird is for a north american to think about it... Imagine, until a few years ago I remember some gypsys coming to my mom's little village in Spain with a portable cinema. We saw old movies sitting on the ground of the village main square... Adorable! :)

How could you not notice it? And the unibrow too! Very distracting.

All devised to make her more unattractive. The characters are broadly drawn to make them funnier and exaggerated. Rosalia is portrayed as an uneducated, unsophisticated girl. The unibrow and moustache complete the look.
