From the hull number (363) the actual boat used was SS-363 USS Guitarro, Gato-class submarine:

Commissioned: 26 January 1944 Manitowoc, Wisconsin, Lt. Cmdr Enrique D. Haskins
War Patrols:
-1st: 7 May 1944 Pearl Harbor, 27 June Fremantle, Australia
-2nd: 21 July 1944 Fremantle, 8 September 1944 Fremantle
-3rd: 8 October 1944 Fremantle, 16 November 1944 Fremantle
-4th: 11 December 1944 Fremantle, 15 March 1945 Fremantle
-5th (Final): 9 April 1945 Fremantle, 8 June 1945 Pearl Harbor

Arrived San Francisco 18 June 1945. Decommissioned & placed in Reserve 6 December 1945 Mare Island, Vallejo, CA

Recommissioned 6 February 1952 for training off San Diego, CA until again Decommissioned 10 September 1953. Then fitted with a fleet snorkel at Mare Island. Recommissioned 15 May 1954 for training Turkish sailors under Military Defense Assistance Program. Again decommissioned 7 August 1954 and loaned to Turkey. Formally sold to Turkey 1 January 1972.

Received 4 battle stars and a Navy Unit Commendation during WWII in the South Pacific.
