After the bullet was fired into the door jam, wouldn't a new round automatically be inserted in the barrel? If so, why was there a clicking sound and no shot when his wife grabbed the gun?
The doctor had secretly removed the bullets while checking the bullet hole. After he grabs the gun back from his wife he quickly reloads the gun. He was testing his wife to see if she was capable of killing him, which she was.
