Does it seem plausible that a train could be derailed by a little brush on the tracks, as was done by Lewt?

The little bit of brush on the tracks was covering the fact that Lewt had opened up the rail.

Although, to be fair, the track was on a bend and he unbolted and moved the inside rail further to the inside of the curve - the result shouldn't have been a derail, and certainly the train wouldn't have fallen to the inside of the curve.
How could you have possibly missed the moved rail?
Perhaps the OP just wants to reach out for some sense of community.
