I assume that Helen parked her car farther away and Ann didn't see any car but the one Marriott and Marlowe came in.
Marlowe convinces Moose that Amthor double-crossed Moose (which is true) and Moose acted on that conviction: he kills Amthor for the betrayal. I think Moose also still wanted Marlowe to find "his Velma".
Marriott was beaten to death by Helen. Either she sneaked up on him before hitting him, or he let her get close because he knew her. Then, when he turned his head, she struck him with a blackjack repeatedly. Her strength - or lack thereof - is mentioned in the film, because Marlowe talks about how sloppy the kill was, thanks to the killer's apparent lack of strength, needing to hit Marriott multiple times.
